• Respiratory School Attended
• Year of Graduation
• CSRT Registry #

Membership:
Liability Insurance for professional negligence (in the amount of $2 million or greater)
Proof of Criminal Record Check (hard copy to be submitted with application)

Employment Status:

• Current Primary Employer
• Manager's name

Fee:

• Online payment will be processed to complete the registration application.
